The Challenge
On a Tuesday, a client contacted us with an urgent request: he was being discharged and needed to move to Brisbane by Friday to attend a family gathering. Typically, we require two weeks to arrange such transitions. However, given the client’s tight timeline, we sprang into action.
Our Solution
We identified a vacant tenancy in Capalaba that met the client’s needs but required minor modifications for wheelchair access. By leveraging our supplier network, we custom-built a ramp just a day before the client’s arrival.
Staff and Technology
The client had specific manual handling needs. Our experienced staff, skilled in using hoists, Sara Steadies, hospital beds and other assistive technologies, were ready. We secured necessary assistive devices, including a hospital bed, shower commode, wheelchair, and sit-to-stand chair, some of which were short-term hires replaced later with custom solutions.
Post-transition, our team ensured the client’s entitlements and pension were set up seamlessly, leveraging their expertise to navigate the process smoothly.
Understanding the critical nature of stroke recovery, we engaged a physiotherapist with NDIS experience in stroke rehab. A detailed rehab plan was implemented and our support team ensured daily exercises were carried out, aiding the client’s recovery.
Community Integration
To prevent social isolation, we facilitated community integration by hosting events within the local disability community, helping our clients build relationships quickly.
