Client-Focused Supports – Reducing complex and challenging behaviours through NDIS Choice and Control.
At Integrate Disability we focus on providing NDIS supports that prioritise choice and control, leading to improved capacity building and behaviour management in Supported Independent Living (SIL) environments. This case study highlights our successful approach in reducing complex behaviours for a client through personalized support and community integration.
Supported Independent Living Case Study
We were contacted by a client’s support coordinator to assist a male with a history of aggressive behaviour in our local area. The client expressed frustration over the lack of choice and control in his current support system. Our goal was to address these issues and provide a supportive environment that met his specific needs.
Key issues.
Accommodation: In consultation with the support coordinator and public trustee, we identified the criteria for a suitable home. Leveraging our network of local real estate agents, we found a property and negotiated the installation of air conditioning by the landlord.
Furnishing: We fully furnished the home in a modern contemporary style, consulting with the client to incorporate their passions into the decor, making the property feel like their own home.
Behavioural Support: The client had complex behaviours stemming from past trauma. Working with the allied health team, we established a team of skilled support workers specifically trained to assist this client. We also respected the client’s preferences for support workers.
Centrelink Transition: Our experienced support workers facilitated a seamless transition to the new property, ensuring the client continued to receive all entitlements. We scheduled and attended a Centrelink appointment with the client.
Financial Management: Collaborating with the Support Coordinator, we ensured the costs of support services, housing, and board were within the client’s budget, maintaining their dignity and choice while delivering high-quality services.
Community Integration: Operating primarily in the Redlands LGA, we connected the client with our existing social events to foster a sense of community. With the client’s permission, they began participating in these events, which they had been missing for a long time.
Personalised Support Plan: We developed a plan to move the client into their own home, assembling a team of highly skilled support workers tailored to their needs. The client was initially sceptical due to past experiences with NDIS but soon appreciated our stable roster of trusted support workers.
Maintaining Medical and Allied Health Services: Reassuring the client, we continued engaging with their existing allied health and medical team, ensuring continuity of care.
The client has successfully settled into their new home, experiencing a significant reduction in incidents and aggressive behaviours. They have taken pride in their home, actively participating in capacity-building activities such as doing dishes, laundry, and house and yard maintenance
