Transitioning a client into our support care

Case Study

We were approached to take over support for a man with a disability living in Cleveland. He had been with his previous provider for over 10 years but faced challenges due to complex behaviours and difficulty maintaining a stable support team. The previous provider, despite their best efforts, could no longer service the client, prompting his family to reach out to us for assistance.

The NDIS Transition Challenge:

  • The client had complex behaviours with regular emergency services involvement.
  • The client was living in a home rented and furnished by his previous provider.
  • The client did not want to relocate and was happily settled in his home.
  • The client did not want to share their home with anyone else.

 

Our Transition Solution
1 Tailored Training and Team Introduction: We collaborated with the existing allied health team to receive specific training tailored to the client’s needs. Involving the client in the transition process allowed him to meet the new support team beforehand, easing the transition.

2 Securing the Disability Living Accommodation: To maintain stability, we negotiated with the previous provider to take over the rental agreement and purchased all the existing furniture. This minimised environmental changes, reducing stress for the client. We secured the rental for an additional 12 months, ensuring continuity in his living situation.

3 Custom NDIS Support Model: Given the client’s preference to live alone, we worked closely with his allied health team, support coordinator, and the client himself to develop an NDIS support model that met his needs, respected his preferences and stayed within budget.

The NDIS Transition Result – The client has experienced a significant reduction in complex behaviours and has been happily living in his home for nearly two years. He has become an active participant in the community and is making considerable progress towards his personal goals, with no intention of relocating.
